    Based on 20 interviews, this qualitative hermeneutic thesis examines the motivation of elderly plastic surgery patients, who plan a major intervention (facelift) or have already undergone surgery. The results of the study indicate that the current standard procedure neglects the psychological risks and side effects of such an operation. After taking into consideration the patients' conscious motives and the social context of their decision to have surgery, this paper focuses on the complex issues of personal self-image, world view, emotionality and neurotic fixations. The high incidence of maladaptive coping, narcistic self-esteem management, low social-emotional competence, lack of meaning in life and the tendency towards self-deception makes the indication for surgery appear problematic. Weighing the benefits against the risks of surgery from an psychotherapeutic viewpoint, it seems doubtful whether a cosmetic intervention can solve the patients' problems or may even do more harm then good under unfavourable conditions. A mandatory psychological counselling and information session prior to surgery could take into consideration and minimise this fundamental psychological dilemma. The study concludes with a proposal of a procedure that combines psychological and surgical intervention for a holistic therapeutical concept to deal with the patients' aging anxiety. This innovation would be beneficial to both the patients and the surgeon in charge.
