Craniomandibular dysfunction (CMD) is characterized by an unconscious overload of the masticatory muscles and is viewed as a multifaceted clinical picture with highly variable pain symptoms as well as dysfunction and dyscoordination disorders of the temporomandibular joint (Ahlers and Jakstat 2011; Schindler and Türp 2009). In this context, intramuscular botulinum toxin injection into the lateral pterygoid muscle is used as a causal therapy (Majid 2010). At the Clinic and Polyclinic for Oral and Maxillofacial Surgery at the University Medical Center Greifswald, a botox injection into the lateral pterygoid muscle - in contrast to other clinics - using realtime surgical navigation is offered as an optional service for patients with CMD and pronounced temporomandibular joint myopathy. In the present study, this therapy was assessed retrospectively. For this purpose, 17 patients (mean age 53.6 years, 88.2% women) and 18 patients (mean age 52.3 years, 77.7% women) with CMD and pronounced TMJ myopathy were examined in the injection group. The injection group received 25 units of Bocouture® per lateral pterygoid muscle. Three depots of 8.3 units each were injected per muscle using real-time navigation (Vector Vision®, Brainlab) at the muscle origin, belly and insertion. The control group received no botulinum toxin injection because the patients had not accepted the offer of this optional service. The follow-up examination was carried out according to the Research Diagnostic ...
